Flexible Person-centered service to assist persons thrive in the community.
Combines both training and support. Can be delivered either in-person or remotely.
Supported Living Services (SLS)
Services provided to a person who requires daily staffing support for challenging behaviors, medical needs, physical needs, and/or life skills.
Individual Community Living Supports (ICLS)
Group of services for people on the Elderly Waiver or Alternative Care program who need reminders or a moderate-level of assistance in their home.
24-Hour Emergency Assistance (Waiver)
On-call or in-home problem solving, counseling, companionship, or emergency assistance for people in their home or community
Night Supervision
In-home, awake, overnight help for people with disabilities.
In-Home Family Support
Support to help individuals and their families work towards their goals.
Respite
Temporary services provided so that the primary caregiver can take a break.
Community Residential setting
A safe place where residents call their home and enjoy 24 hour care. Call to check availability and location.
Transportation Services
